Online Form Submissions

 

In you would like to begin using the mailbox assigned to your lot, please complete this form and then pay for the box in the Payments section. This form will be emailed to our property manager, Trish Svejkovsky. Contact her to pick up your keys. Her number is 928-369-4000.

 

Once you have your keys, you must go to the Lakeside post office with your ID and inform them that your mailbox is active.
